Upon submission of interest, you will be directed to complete a Screening Questionnaire in our patient portal. Please note we use our electronic portal heavily with our client and patient population, and an inability to navigate the online portal is incompatible with our model of care.

Current patients of Dr. Emily D’Adamo from the University of Bridgeport School of Naturopathic Medicine Clinic are eligible for legacy pricing to allow for continuity of care following the closure of the clinic.
