How do I get started?

There are two ways you can schedule with the Center for Generative Medicine. You may use our Online Scheduler below, which links to our Charm Health portal. Or, you may fill out the form at the bottom of the page to contact us and begin scheduling. We will respond to your request within 48 business hours, but please be sure to check your spam section of your email!

Who should I schedule with?

If you are interested in working with us virtually, please schedule with Dr. Jennifer Goto, our Director of Telehealth.

If you are looking to visit our Norwalk, Connecticut location, please schedule with Dr. Emily D’Adamo.

Please note that Dr. D’Adamo is not taking new virtual clients at this time, and virtual visits will be scheduled with Dr. Goto.

What is the pricing structure?

  • Initial consultation $350
    Prior to this visit, we will ask you to submit any recent labs, scans, or medical documents that you may have, as well as fill out a questionnaire regarding chief concerns, medical history, current medications or supplements, and things you would like us to focus on. This allows us to research and prepare adequately for our  visit and use our time together effectively. During the conference, we will get to know you better as an individual, gather more information, and answer any questions you might have. At the end of the conference, you will receive your personalized diet and supplement protocol.

  • Follow-up consultation $275

    After your first visit, we'll schedule a progress check to examine your response to your personalized plan. After your first encounter, you're officially onboarded and are able to reach out via our patient portal with questions and schedule appointments whenever you'd like.

    Please note that we bill all appointments on the morning of the scheduled visit. If you need to reschedule or make a cancellation, we ask that you notify us 48 hours prior to the appointment, otherwise a 50% late fee is incurred. A no-show will incur the full amount for the scheduled appointment. Thank you for your understanding.

    You may elect for only traditional consultation, any additional testing or analytics are add-ons. 

  • Genomic Analysis $1095 (Test kit included. The price of the test, $270, is charged at the time of shipment) No Genomic Analytics are completed during the initial visit.

    We take the data collected from a salivary sample to organize and interpret your genomics. This one-time collection allows us to employ precision analytics to your current and future state of health. This will allow us to assess the genes that affect your ability to make and break down neurotransmitters, and hormones, as well as detox, thyroid, inflammatory pathways, and more. Additionally, we can create a diet that is completely personalized to you, based on your specific genetics. Patients receive a complete SWAMI23 nutritional plan at the end of their visit and are able to use genomic analysis for all future appointments and concerns without additional charge.
